Minimum Qualifications
NEED TO HAVE QUALIFICATIONS:
Bachelor’s degree in Social Work, Psychology, Sociology, or related Human Services field
AND
One year full-time professional experience in a social services agency OR Two years full-time paraprofessional experience in a social services agency.
AND
Valid Driver’s License

NICE TO HAVE QUALIFICATIONS:
Master’s degree in Social Work, Psychology, Sociology, or related Human Services field
Previous experience in completing initial screening and assessment in one or more of the Adult Service Program areas: Adult Protection, Chemical Health, Mental Health, Housing, and Pre-Petition Screening.

Duties & Responsibilities
These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position’s management approved work schedule is required for all positions.

1. Effectively engage individuals and their families to assess and evaluate needs and strengths, including safety and risk.
2. Determine with the client, and in collaboration with agencies, eligibility, need, resources, and level of service for identified clients.
3. Provide immediate response in crisis situations. Assessing for immediate safety and risk areas, in collaboration with Crisis Teams and community partners, to safely plan to minimize personal and community risk.
4. Demonstrates a commitment to support cultural diversity County wide, actively supports the County’s inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities.
5. Arrange and coordinate direct services, resources, and advocacy to clients and families.
6. Facilitate, integrate, and mobilize agency and community resources on behalf of the client.
7. Evaluate, document, and monitor progress toward goals and objectives at an intake level, referring to appropriate services, resources, and supports.
8. Research, maintain, and apply knowledge of relevant policies, procedures, and statutes. Provide training and education to others.
9. Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.
10. Maintain appropriate engagement and boundaries with clients and colleagues.
11. Perform other social services duties as assigned.

ESSENTIAL JOB FUNCTIONS: Duties 1-10 are essential functions.
Knowledge, Skills & Abilities and Work Environment
• Knowledge of child and human development, family dynamics and systems, including crisis management and cultural competency.
• Knowledge of community resources and funding sources.
• Knowledge of medical diagnoses, medications, and basic medical terminology, as well as medical insurance processes and procedures.
• Knowledge of statutes, rules, and policies pertaining to social work and core job functions (e.g. Vulnerable Adult Act, Rule 79 Adult Mental Health Case Management)
• Interviewing, negotiation, and listening skills.
• Ability to communicate effectively both orally and in writing.
• Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English.

WORK ENVIRONMENT: Work is performed in a County office building answering calls and in person interactions. Lifting requirement of up to 20 pounds on an occasional basis. Equipment used may include, but is not limited to, computer, printer, telephone, cell phone, and copy and fax machines.
