ESSENTIAL JOB DUTIES

Under direction, provides day-to-day supervision to QIDPs which includes monitoring and verifying proper documentation, ensuring quality service is provided and delivering goals. Trains and directly work with QIDPs in performing a variety of case management work in accordance with established policies, procedures and conditions required by regulatory agencies. Oversees the Vocational Rehabilitation Program in the absence of the Vice President.

  • Supervises the day-to-day activities of the QIDPs ensuring compliance with policies, procedures, guidelines, regulations; implements changes recommended by Commission on Accreditation of Rehabilitation Facilities (CARF), Department of Mental Health (DMH), Department of Human Services (DHS), and Department of Public Health (DPH).
  • Establishes performance standards and objectives for QIDPs; directs work allocation, schedules, processes, organization, prioritization, and quality.
  • Recruits, trains, coaches, disciplines and evaluates QIDP employees’ performance; documents and communicates performance and behavioral concerns to Human Resources in a timely manner.
  • Reviews reports, resolves problem and motivates employees to achieve optimal production and performance.
  • Develops and maintains positive relationships with relevant community resources, including funding sources, residential facilities, community agencies, and family members of service participants.
  • Provides technical/professional coordination and leadership in the execution of daily program activities pertaining to QIDPs as appropriate to program objectives and area of expertise.
  • Monitors and assesses QIPDs paperwork to ensure accuracy and completion of work within established timeframes; complies with standards established by CARF, DMH, DHS, and DPH guidelines for the Vocational Rehabilitation Program.
  • Acts as the liaison between QIDPs and the VP of Vocational Rehabilitation, external programs, and community resources in order to achieve program objectives, utilize available resources, and enhance team efforts.
  • Works in collaboration with an interdisciplinary team in the development, implementation and revision of Individualized Service Plans.
  • Designs and develops service participants’ Integrated Service Plan/Vocational Summary with input from the interdisciplinary team.
  • Assesses with specified appraisal tools and analyzes scores and data about assigned service participants within the first 30 days of programming; repeats process annually for service planning.
  • Maintains assigned service participants’ case files and program records as specified by applicable standards, regulations, and funding requirements.
  • Oversees the activities of the Vocational Rehabilitation Program in the absence of the Vice President
  • Consults and cooperates with community systems to facilitate linkage, referral, crisis management, and advocacy; follows up with the focus on attaining program goals.
  • Creates access, builds credibility, establishes rapport and maintains communication with various community services and resources.
  • Works with service participant to explore career opportunities.
  • Transports service participants to and from home, job sites, meetings and other appointments.
  • Manages and oversees service participants in their designated program settings.
  • Works in conjunction with direct service staff on programming and curricula for service participants in the classroom and assists in the classrooms as needed.
  • Regularly reviews participant case files to stay up to date on their service plans.
  • Records and monitors maintenance skills.
  • Maintains knowledge of industry trends, policies and procedures.
  • Builds trust by interacting with others in a way that gives them confidence.
  • Communicates the organization’s mission, vision, values and promote diversity.
  • Demonstrates leadership and positive role modeling for staff.

REQUIREMENTS

  • Requires bachelor’s degree in Psychology, Rehabilitation, Counseling or a related field.
  • Requires three years of experience in working with persons with developmental disabilities, including one year working as a QIDP, and one year supervisory experience.
  • Requires experience in collaborating with community resources and/or state or accrediting agencies.
  • Requires knowledge of the characteristics and concerns associated with the population served.
  • Requires CPR/First Aid certification or ability to obtain certification within six months of employment.
  • Requires strong interpersonal, effective verbal and written communication skills; ability to work with a wide range of people.
  • Requires knowledge of de-escalation techniques to prevent and minimize upset/ aggressive reactions.
  • Demonstrates working knowledge of Microsoft Office products including Word, PowerPoint, Outlook, and Excel, and internet navigation.
  • Requires use of personal/company vehicle to transport service participants and/or equipment.
  • Requires valid driver’s license, good driving record and proof of insurance.
  • Requires ability to meet and maintain the credentials needed for a QIDP.
  • Maintains confidentiality of information related to LLGI operations, financial matters and personnel matters.
  • Interacts with people in a manner which enhances their dignity, privacy and confidentiality.
  • Requires ability to develop strong trusting relationships in order to gain support and achieve results.
  • Requires ability to compile information and write reports.
  • Requires excellent organizational, problem solving and collaboration skills.
  • Requires a high level of professionalism, integrity, business conduct and ethical behavior.
