Description
Position Summary: A LINK Case Manager’s primary role is to provide intensive in-home crisis intervention for youth who are in crisis and their families, through assessment of child needs, goal plan development, counseling and skill development. The LINK Case Manager is also responsible for linking the youth to long term therapeutic and recreational community based supports, providing advocacy, monitoring and consultation of the youth’s needs.
Essential Accountabilities:
- Responsible for assigned caseload working with the identified youth and their families for a period of 4 to 6 weeks providing intensive services in order to maintain youth within their homes and communities and avoid an escalation in level of care needed.
- Responsible for educating families regarding children’s mental health issues, teaching behavior modifications techniques and parenting skills and providing parents with crisis stabilization strategies.
- Provides families with appropriate linkages and assists with relevant community resource identification. The worker acts as a professional representative, of the program and agency, when working in the community to collaborate with schools and other organizations.
- Responsible for all paperwork related to cases, including progress notes, intake forms, releases, goal plans, termination summaries, etc.
- Provide on call crisis intervention services on a rotating basis.
- Participates in monthly team meetings, supervisions, case conferences, etc.
- Other duties as assigned.
Qualifications
Bachelors degree and one year experience in the Human Services field required. NYS drivers license and access to a vehicle.
