POSITION SUMMARY

We are seeking a highly motivated and experienced Nurse Practitioner Manager to oversee and manage our in-home risk assessment program. The ideal candidate will be responsible for leading a team of nurse practitioners and ensuring that all in-home risk assessments are conducted in a timely and efficient manner.

DUTIES & RESPONSIBILITIES

  • Establish strategic goals by gathering pertinent business, financial, services, and operations information aligned with the organization.
  • Conduct in-home assessments on identified patients, following established guidelines and necessary training.
  • Oversee and manage a team of nurse practitioners who conduct in-home risk assessments both in person and via telehealth.
  • Maintains staff by recruiting, hiring, onboarding and ongoing training and education of employees.
  • Develops personal growth opportunities.
  • Coaches, counsels, and disciplines staff when necessary. Provides performance evaluations and assesses competency annually. Develops and monitors corrective action plans as necessary.
  • Develop positive relationships with clients, caregivers, practitioners, and other team members.
  • Develop and implement policies and procedures for the in-home risk assessment program to ensure compliance with all regulatory requirements.
  • Acts as a mentor and support person to the nurse practitioners and other staff members to Identify and intervene for urgent and emergent situations.
  • Educate and provide clinical direction for staff on topics related to health conditions and clinical documentation.
  • Coordinate with other departments to ensure that all necessary equipment and supplies are available for the in-home risk assessment program.
  • Utilize chart reviews to identify potential documentation gaps and identify opportunities for improvement.
  • Conduct Risk Adjustment improvement initiatives, develop recommendations for Risk Adjustment remediation plans, and create tools and databases to capture relevant data for assigned markets to achieve company and market-specific Risk Adjustment goals and initiatives.
  • Provide actionable, measurable solutions to providers that will result in improved documentation and coding accuracy, optimal suspect closure, and quality gap closure.
  • Provide education and training to nurse practitioners and other staff members on best practices for conducting in-home risk assessments.
  • Participate in Quality Assurance Process Improvement.
  • Collaborate with other healthcare providers to ensure that our members receive the best possible care.
  • Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).
  • Assumes responsibility for personal growth.
  • Develops, maintains, and upgrades professional knowledge and practice skills through attendance at seminars, conferences, and participation in continuing education and in-service classes.
  • Travel may be required for up to 25% of the position. The regions are identified and updated based on business needs and may include the training and oversight of staff.

SKILLS & QUALIFICATIONS

  • 5+ years of Clinical Experience preferred.
  • 2+ years of experience in a leadership role in a healthcare setting.
  • Strong knowledge of Centers for Medicare & Medicaid Services (CMS) regulations and guidelines.
  • Experience in conducting in-home risk assessments.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Proficiency in electronic medical records and other healthcare software systems.
  • Skilled in performance management, quality management, project management, coaching, achieving results.
  • Computer skills proficiency required: (Microsoft Office Suite).
  • Ability to handle change and rapid growth.

EDUCATION, LICENSES, & CERTIFICATIONS

  • Current licensure as a nurse practitioner in Arizona. Willingness to get other state licenses as necessary.
  • Master’s degree in nursing.
