Description
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Homebase Navigator is responsible for preventing homelessness through the use of a program model designed for custom tailored service provisions, for each household entering the program. They will work with the entire RiseBoro Homebase team to steer all efforts, ensuring that every household: has a truly holistic service plan; is supported throughout their housing crisis; has their short-term and long-term challenges addressed; and does not enter shelter. They will facilitate the continuum of care from Outreach to Reception/Intake to Navigation by:
- Engage in researching and planning for every scheduled household, utilizing available information;
- Keep abreast of all resources available for Homebase clients including HRA policy and procedures, RiseBoro protocol, rental assistance programs, legal service providers, external resources, etc. – to ensure excellent service provision;
- Triage households and conduct an Risk Assessment Questionnaire to determine the level of service;
- Make recommendations to Case Managers in order to create and implement a comprehensive services plan, based on the case type, service type, RAQ, household composition, housing crisis, and extenuating factors;
- Determine the appropriate internal and external referrals for each case and every member of the household;
- Assess each household for long-term City, State and Federal rental assistance programs;
- Relieve the clients of trepidation and serve as the voice of knowledge in each case, regardless of complexity;
- Explain the RiseBoro Homebase process and provide compassion, reassurance and Homebase staff accountability for each household;
- Assign each case to a Case Manager that specializes in that specific service type (prevention, relocation, community support, diversion);
- Devise a Critical Time Intervention based model service plan for all intensive Homebase cases;
- Utilize a warm handoff technique to ensure that each client feels confident throughout the process;
- Assess, support and supervise the Case Management Team throughout the delivery of the service plan;
- Review or assist with the completion of every Financial Assistance Request for households enrolled under your directive;
- With assistance from the Homebase Director, provide supervision to Case Managers for the duration of the case revising plans and case conferencing, when necessary;
- Assist Homebase staff with general program troubleshooting and improvement strategies;
- Work with the Homebase Director to develop a collaborative approach with employees to accomplish the programs objectives;
- Facilitate meetings and trainings with staff to develop best practices in the program and ensure effectiveness of operations;
- Participate in the ongoing development of the Homebase program and Community Empowerment Center (CEC);
- Maintain accurate record-keeping via case files and the designated computer database;
- Develop and implement improved case management strategies for program participants;
- Build and maintain relationships with partner agencies;
- Ensure that program participants rights are protected;
- Other duties as assigned by Homebase Directors and Executive Management.
COMPETENCIES
- Current RiseBoro Homebase Case Manager
