Description

Under the direction and supervision of the Program Managers for Victory Wellness and Valley Employment Services, the Rehabilitation Liaison (RL) is responsible for providing pre-vocational services and working collaboratively with Center staff, VES staff, and Department of Rehabilitation (DOR) staff in order to transition consumers effectively to services provided through both Valley Employment Services (VES) and the Department of Rehabilitation (DOR). Additionally, to work collaboratively with Center staff and each consumer in order to develop service goals, with the overall goal being obtaining and maintaining employment, leading to self-reliance and self-sufficiency. The RL serves as the “point” of contact with VES regarding case coordination. JOB Rehabilitation Liaison -VWC ID#35235A

DUTIES AND RESPONSIBILITIES:

  • Provides pre-vocational services including: helping consumers identify interests and strengths; exploring stages of work readiness and translatable skills; conducts financial assessments; addressing “on-the-job” disclosure (s) and job accommodation; exploring job preference and job tolerance; and promoting the value of “work” in creating self-sufficiency, self-reliance to consumers. Additionally, the RL “fast tracks” the Vocational Assessment process in order to help consumers identify employment goals.
  • Receives, reviews, and tracks all incoming referrals. The RL informs the DOR Counselor regarding pending referrals. Additionally, the RL works with the DOR Counselor in scheduling the consumers initial intake appointment with DOR. The RL also participates in the initial intake with the DOR Counselor and the consumer.
  • The RL gathers the necessary documents and helps the consumer complete the Cooperative Agreement Referral Packet.
  • Communication/correspondence related to the consumer’s services and progress are shared and documented in a case note written by the DOR Counselor and RL/VES Employment Specialist staff once a month at a minimum. The RL and VES staff coordinates the closure/de-activation of cases.
  • Completes all chart documentation within the timeframes established by the Center, DOR, and the Los Angeles County Department of Mental Health.
  • Works collaboratively with VES Employment Specialist staff in the development and completion of the Individualized Plan for Employment (IPE).
  • Works collaboratively with Center staff and each consumer in creating a smooth and seamless transition to DOR services. Serves as the “point of-contact” between VES and DOR.
  • Attends VES case conferences in order to exchange information and coordinate services.
  • Attends weekly supervision meetings.
  • Attends meetings as assigned (e.g., DOR Quarterly Meetings and Center’s Employment Steering Committee Meeting).
  • Attends in-service training (s) as assigned.
  • Performs other duties as assigned by the Program Manager (s).

Qualifications

  • The ideal candidate must have a Bachelor’s degree in Social Work, Psychology or related field desired.
  • Knowledge of chronically mentally ill population, crisis intervention techniques, community resources, Social Security and GR benefit packages.
  • May be required to have a current California Driver’s License and be insurable by the Center’s auto insurance carrier.
  • Ability to speak Spanish is desirable for some positions in this classification.
