Description

Assist the Program Director in the daily operation of the residence; this includes hands-on activities. Ensure the quality of care for all residents. Supervise residential staff. Ensure program and case records are in compliance with agency and DOHMH regulations. Interface and maintain written communication with other service providers and agency support staff.

ESSENTIAL ACCOUNTABILITIES:

  1. Assist the Program Director with the hiring process of all staff, reviewing resumes, setting up and conducting interviews.
  2. Orientation / training of all staff.
  3. Document supervision of staff, providing formal, bi- weekly direct supervision to Family/Housing Specialist, Peer Specialist and Direct Support Professionals.
  4. Completion of annual staff evaluations. Progressive discipline of employees as necessary.
  5. Oversees schedule to assure appropriate shift coverage.
  6. Oversees all file documentation. Periodically does internal audit of files and review tracking form.
  7. Co-facilitates monthly staff meetings.
  8. Facilitates monthly meetings with overnight staff.
  9. Responsible for ensuring compliance with DOHMH regulations.
  10. Responsible for revenue monitoring and reporting.
  11. Responsible for overseeing, identifying, and reporting of physical plant safety issues. Interact with General Service personnel.
  12. Responsible for completing payroll
  13. Conduct random spot checks of all shifts to ensure effective operation of shifts.
  14. Responsible for maintenance of activity funds and petty cash funds.
  15. Participate in integrated activities, i.e. social / recreational activities and cultural events.

OPERATIONAL

  1. Facilitate communication between program and the Intake Department to ensure maintenance of wait list.
  2. Evaluates treatment and care of residents and monitors records. Oversees program self-audit process.
  3. Conduct weekly case reviews with residential staff
  4. Review all referrals and determine eligibility for admission.
  5. Oversees Residence staff towards development and implementation of Service Plans. Facilitates communication between all staff towards assisting resident in attainment of Service Plan goals.
  6. Oversee discharge process. Supervise the implementation of the discharge plan.
  7. Responsible for Consumer Evaluation process (HRA/SPA application)
  8. Ensure ongoing training and development of staff.

OTHER RESPONSIBILITIES

  1. Participation in the development and implementation of program trainings.
  2. Attend workshops and conferences as requested. DOHMH mandatory trainings required.
  3. Transport residents with agency vehicle as needed.
  4. Responsible for maintaining (or having access to) a personal vehicle throughout the course of the workday.
  5. Other duties as assigned.

Qualifications

Bachelors Degree in Social Services with three years of experience working with adults and children with behavioral health or other disabilities or Associates Degree with five years of experience. High School Diploma required. Three years work in human services required. Supervisory skills preferred. New York State drivers license and access to a vehicle required
