Description

Provide substance abuse and mental health services focusing on the treatment, prevention and awareness of opioid, alcohol and other substance addictions as a member of a multidisciplinary health team.

Essential Functions

  • Expand mental health services and substance abuse services focusing on treatment, prevention and awareness of alcohol and other substance use.
  • Provide mental health services utilizing PCBH/SBIRT model including completing Diagnostic Assessments and brief interventions.
  • Seeks to understand and meet the needs of customers (patients, families, vendors, coworkers) through respectful, courteous and culturally sensitive interactions.
  • Develop and maintain a consistent model of prevention and intervention and counseling in chemical dependency based on relevant research and clinical experience.
  • Take initiative in program goal setting, evaluation and problem-solving effective approaches to chemical dependency activities.
  • Ensure engagement and support through a patient centered approach to care.
  • Provide outreach, assessment, education, intervention, referral and limited case management to individuals and groups.

  • Maintain network associations with Ramsey County Mental Health Center. Ramsey County Chemical health and unit and other agencies to coordinate outreach efforts and with treatment programs and hospitals.
  • Coordinate client care with mental health and social service staff to maximize successful outcomes.

Work Environment
Primary environment is home office, administrative office, or clinical office. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses.

Physical Demands

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.

Qualifications

Required Education and Experience

  • Bachelor’s Degree in related field
  • Licensed Alcohol and Drug Counselor license
  • Understanding of, and comfort with, harm reduction focused interventions
  • Knowledge of chemical health treatments
  • 3 years’ experience working in the mental health field

Preferred Education and Experience

  • MSW or equivalent (LICSW, LMFT, LPCC), LGSW or LICSW preferred
  • 5 plus years’ experience working in the mental/behavioral health field

Additional Eligibility Requirements

  • Demonstrated success in working effectively with target population(s).
  • Change Agile; ability to operate in the gray and flex to new developments or situations.
  • Experience working in a multi-site environment is highly desired.
