For 80 years, we have been one of upstate New York’s largest providers of programs and services for individuals who have disabilities. Many of the innovative programs and vital services that we offer are not available elsewhere.

We are searching for an Intake and Enrollment Specialist to join our agency.

The Intake and Enrollment Specialist is the initial contact with potential individuals, their families, and Care Managers, seeking services, specifically in Residential Services. The Intake and Enrollment Specialist will conduct screenings and manage the intake and enrollment processes, while ensuring both internal and state practices are adhered to. The position requires working closely with Residential Management and outside organizations, including OPWDD and Care Coordination.

Responsibilities:

  • Responsible for responding to initial requests for information regarding services and provides linkage and referral to CFDS services accordingly
  • Maintains current information on Center programs and services and community resources
  • Complete screenings and assessments of individuals needs to make recommendations to program management
  • Build connections for those seeking services with other families already receiving services
  • Develop, maintain, and oversee the intake process, policies, and procedures
  • Responsible for coordination and follow up from initial contact through the intake process
  • Identify placement and appropriate program settings for individuals
  • Ensure all documentation, policies, and procedures are followed and completed through the intake and enrollment process

Requirements:

  • High School Diploma or equivalent is required
  • At least 3 years of job related experience, specifically in the field of Human Services
  • Ability to handle multiple tasks simultaneously, effectively communicate, make decisions and problem solve
  • Flexible scheduling to meet the needs of individuals and their families
  • Reliable transportation to travel to locations throughout the greater Capital Region area

The compensation range for this position is $21.78 – $23.78. Rate of pay is commensurate on experience.
