Description
SUMMARY:
The Health Education and Access Coordinator will help assess individual and community health education needs related to Harm Reduction and Overdose Prevention; plan, implement, and evaluate effective health education programs; coordinate health education services; serve as a resource person in health education. The goal of the Health Education and Access Coordinator is to provide people who are at high risk of Overdose with tools that allow them to prevent developing life-threatening health issues.
- Develops and maintains relationships with community members and leaders to assist in gaining program support, referrals, and case assistance to help identify individuals at high risk of overdose within the community.
- Performs street and community outreach for Harm Reduction Education program promotion and case finding. Attends community related events in NYC area.
- Conducts individualized and/or group health education around Harm Reduction and support coordination to connect individuals to their Primary Providers for Shared Decision Making.
- Maintains chart for each encounter; clinical care supervised by collaborating clinician.
- Conducts follow-up with patients as indicated.
- Assesses individual and community needs for health education on an ongoing basis.
- Works with the Care Management Coordinator to develop/select/adapt and implement a health curriculum and promotion materials to be delivered to community members.
- Evaluates effectiveness of curriculum and revise as needed with implementation of current best practices.
- Maintains records and files.
QUALIFICATIONS:
Education/Licenses Required: Bachelor’s degree in social work, psychology, public health, health education or related field.
Experience and/or Skills
- Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
- Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time.
- 1-2 years’ experience in health education.
- Ability to generate, interpret and analyze data from multiple sources.
- Knowledge of social services, community-based organizations, and other resources in the NYC area
Bi-lingual, Spanish preferred.
