Description

JOB SUMMARY: The Guardian will support the Program in coordination of services to Protected Persons under court authority, by overseeing the safety and well-being of the clients, and to protect and maintain their assets. This can include coordination of medical care, appointments, placement, social engagement, benefits eligibility, and all other aspects of the Protected Persons’ lives, using State statutes and professional standards as guidelines. Clients served in this program are elderly with moderate to severe cognitive impairment where the court has deemed the person as lacking capacity to care for themselves or their finances. The Program office is in Forest Grove, Oregon and serves all of Washington and Multnomah counties. May be willing to consider part-time status if preferred and grow into a full-time position. This position offers an opportunity for advancement in the Program.

POSITION TITLE: Guardian (Specialist II)

HOURLY WAGE: $22.31 – $24.58

POSITION STATUS: Full time @ 32hrs/week with room to add more hours in the future

LOCATION: Forest Grove, Oregon

ESSENTIAL JOB RESPONSIBILITIES:

  • Coordination of Medical Care: assists the lead guardian(s) in coordination of the protect person’s medical care, including making and attending appointments, researching medication effects, assuring proper medical insurance coverage, making medical decisions, including hospice and end-of life decisions.
  • Coordination of Care Agency Services: coordinates in-home care-giving services with qualified care agencies to provide in-home care, transportation to appointments, companion care and other services, to provide continued quality of life opportunities.
  • Communication with Service Providers and family: engages in conversations with a variety of providers including doctors’ offices, care agencies, funeral homes, CPAs, attorneys, financial institutions, movers, locksmiths, Aging Services, Adult Protective Services, and other service providers to coordinate needed services to support the Protected Person. This can include applications for Medicaid, or other services available.
  • Coordination of Placement and Moves: assists in the location of appropriate placement into a care facility or home, including touring, completing paperwork, arranging movers and transportation to relocate the Protected Person into a safe environment.
  • Coordination of end-of-life Planning: arrange for funeral plans, cremation services, pastoral visits, end of life care, including hospice services.
  • Documentation and applications for services: documentation of actions taken on behalf of the Protected Person, keeping accurate and detailed records of decisions made, results, and follow-up needed; application for services, including Medicaid benefits. Completes annual guardian reports.
  • Management of assets and finances: approve expenditures for services to the client, assists with financial aspect of client’s needs including bill-paying, shopping, review of assets, income, expenses; coordinates and oversees bids and work with contractors relating to client properties; assistance in completing required reporting to the courts as required. Assists with liquidating of property, and personal items.

Qualifications

REQUIREMENTS AND QUALIFICATIONS:

  • Nationally certified guardian through the Center for Guardianship Certification or ability to obtain within six months.
  • Experience working with at-risk populations including abuse and neglect, particularly those with cognitive impairment (60+ age group).
  • Knowledge of managing personal finances, coordination of medical care, and case management of clients.
  • Understanding and experience with social service programs and services available.
  • Drives own vehicle to provide services to clients and conduct business on behalf of the Program.
  • Ability to solve complex issues resulting in the best outcome for the client.
  • Resourceful in obtaining services for client needs.
  • Familiar with Oregon Statutes (ORS125 Protected Proceedings) and compliance issues.
  • Basic understanding of probate.
  • Strong technology and computer skills.
  • Ability to multi-task with attention to detail.
  • Ability to prioritize tasks to meet required deadlines and client priorities.
  • Capable of working independently on projects as assigned.
  • Ability to work with individuals of diverse backgrounds.
  • Excellent oral and written communication skills.
  • Good organizational skills with ability and willingness to work as an agency team member and support the mission, values, and goals of Impact NW.
