WHO ARE WE?

Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. We implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services, and health care sectors.

As Brilliant Corners has been deemed an Essential Service, Housing Coordinators will be expected to travel throughout Los Angeles County, meet in person with Program Participants, Case Managers/Workers from Partner Agencies as well as Property Managers. Brilliant Corners will provide Personal Protective Equipment (PPE) for all employees providing these essential services.

PROGRAM SUMMARY

The Enriched Residential Care (ERC) program is supported by Housing for Health (HFH) at the Los Angeles County Department of Health Services (DHS) and serves individuals experiencing homelessness who may not be appropriate for permanent supportive housing because they are unable to care for themselves and need assistance with activities of daily living. Brilliant Corners provides intensive case management services, as well as rental subsidy administration and housing retention services, to ERC participants housed in licensed residential facilities across Los Angeles County.

POSITION SUMMARY

The ERC Case Manager will provide intensive case management services to a caseload of approximately 20 individuals with complex needs who are eligible for the ERC program through Housing for Health. The Case Manager will assist clients to achieve and maintain health and housing stability in a facility most appropriate for their needs. The Case Manager plays a critical role in supporting clients to thrive in their new homes through a client-centered and “whatever it takes” approach to services.

RESPONSIBILITIES

  • Accept and process referrals from HFH and conduct outreach to clients and facilities in order to begin enrollment process.
  • Conduct assessments and re-assessments, including a comprehensive psychosocial assessment, in order to evaluate clients’ needs and strengths.
  • Develop and implement individualized case management services plans that ensure housing stability and identify short and long-term goals.
  • Provide support throughout the housing process in collaboration with the internal Brilliant Corners housing team, including but not limited to move-in facilitation, document review, and facility identification for relocations.
  • Provide education to clients on tenant rights and responsibilities as well as eviction prevention counseling and advocacy.
  • Provide linkage to community resources, benefits, and services which may include physical and mental health, substance use treatment, social activities, health insurance, disability benefits, and other sources of financial assistance.
  • Meet with clients frequently and regularly at facilities and in the community, and accompany clients to appointments as needed.
  • Provide crisis intervention and ensure that clients have access to the 24/7 case management crisis line.
  • Provide life skills education and assistance, including budgeting, stress management, conflict resolution, etc.
  • Provide transportation assistance and coordination.
  • Coordinate with clients’ care team, including facility staff, medical providers, housing coordinator, and HFH staff to ensure clients’ needs are being met.
  • Participate in case conferences to review clients’ progress and address any challenges.
  • Collaborate and communicate with ERC partners on a regular basis.
  • Build and maintain partnerships with residential facilities, community organizations, service providers, and public entities to ensure that clients have access to comprehensive services.
  • Maintain updated and accurate client files and documentation.
  • Participate in staff meetings and other agency events as requested.

Requirements

COVID-19 Vaccine: Must have received or be willing to receive the Covid-19 vaccination by the date of hire to be considered. Proof of vaccination is required.

PROFESSIONAL EXPERIENCE

  • At least 1 year of experience providing intensive case management services and a Bachelor’s Degree in Social Work, Psychology, Sociology or a related field OR at least 2 years of experience providing intensive case management services
  • Experience working with individuals with chronic medical and/or behavioral health conditions, ideally with individuals in residential care facilities
  • Experience working with individuals experiencing homelessness preferred
  • Strong interpersonal and client engagement skills
  • Excellent verbal and written communication skills
  • Flexibility regarding scheduling and prioritizing of tasks
  • Willingness to travel and spend significant time in the field
  • Able to transport clients in your personal vehicle
  • Basic computer knowledge (MS Word and Excel required)
  • Familiarity with evidence-based interventions including Harm Reduction, Trauma Informed Care, and Housing First
  • Ability to work independently and utilize critical thinking skills in decision-making situations
  • Good organizational and record-keeping skills
  • Possession of a valid, clean California driver’s license and a personal insured vehicle are required
  • Bilingual in English/Spanish preferred.
  • Travel required: Travel throughout Los Angeles County with approximately 60-70% of the time spent in the field

Salary range for this position is $25.47 – $26.50 hourly. This position is being offered at $25.48 hourly, non-exempt.
