Description

Purpose: (General description and summary)

The Cultivating Community Operations Team Lead is a member of the Cultivating Community Team that provides day to day support for staff and program needs, facilities/operations support, and when required, outreach and support to serve participants who are provided temporary housing at the site. This is a site-based role and cannot be performed remotely.

The goal of the Cultivating Community Team is to provide supplemental support and connection to program participants in collaboration with the treatment team/referral sources to through skills training, case management and care coordination with providers involved, crisis prevention and de-escalation, referrals to substance use services and other supports if needed, community navigation and integration, advocacy, etc. to encourage stabilization and support in preparation for their next step in housing. With additional peer and risk case management support the program aims to minimize participant hospitalizations (emergency department and psychiatric admissions), increase necessary contact between the participant and their respective mental health and substance use service providers, and aid linkage to transitional or permanent housing programs.

Accountabilities: (Responsibilities of the job)

Facilities:

  • Provides operations/facilities support to Program Manager by acting as primary contact.
  • Performs duties and tasks including but not limited to housekeeping, coordination with work orders, county maintenance, contracting and, etc.
  • Monitors site for cleanliness to support a safe environment.
  • Ensures shift responsibilities are completed.
  • Ensures adequate and accurate order of program and general supplies.
  • Coordinates with internal and external providers related to building needs, including updating tracking systems.
  • Engages in creating and establishing policies and procedures related to facilities and safety.
  • Completes and Coordinates inspections for building, OSHA, etc.

Leadership, Teamwork, and Support:

  • Provides day to day support to staff for questions, consultation, and other collaborative and supportive efforts, including but not limited to, staff coverage for site to support increased safety in the environment.
  • Assists in coordination for shift coverage, including possibility of needing to cover shifts.
  • Provides supervision to Direct Support Staff twice per month or as needed in collaboration and consultation with Program Manager.
  • Provides training and onboarding in collaboration with the Program Manager for new staff and orients to program, mission, purpose, values, as well as expectations and job duties.
  • Reviews documentation, IRs, and performance, as well as programming and provides clear and direct feedback to support best practice with person centered, trauma informed, harm reduction, inclusive, lens for supervisees.
  • Maintains budget and technology tracking, and reporting requirements to HMIS/ServicePoint and county in collaboration with Program Manager.
  • Exudes and models New Narrative values and mission in all interactions.

Engagement and Collaboration

  • Model and coaching for staff around skills training, advocacy, case management, community navigation to support caseload participants in working towards their identified goals. If needed, willingness to provide direct support.
  • Willingness to meet and engage with people “where they are,” including their environment and readiness for change.
  • Supports staff in providing opportunities for group activities, skills training, socialization, community integration.
  • Maintains professionalism in encounters with participants and other providers.
  • Must have ability to assist in de-escalation and crisis situations as needed.

Case Management

  • Ability to work in collaboration with emergency services (i.e. police, Project Respond, Multnomah County Crisis, Behavioral Health Unit)
  • Ability to work in collaboration with acute care settings if necessary.
  • Provides support, assistance, and advocacy for linkage to resources and maintenance of resources.
  • Coordinates with providers and resources involved in participant goals and care consistently, including but not limited to weekly contact and care conferences as indicated.

Crisis Prevention/De-escalation/Harm Reduction

  • Provides crisis response and de-escalation in a community-based setting.
  • Ability to complete Risk Assessment and conceptualization of risk and supports.
  • Utilizes crisis prevention approach to decrease use of crisis services, Project Respond, EMS, police, hospitalization, etc.
  • Ability to work in collaboration with emergency services (i.e., police, Project Respond, Multnomah County Crisis, Behavioral Health Unit)
  • Ability to work in collaboration with acute care settings if necessary.
  • Must have ability to assist in de-escalation and crisis situations as needed.
  • Support harm reduction approach, linkage to resources such as, but not limited to: clean needle exchange, NARCAN, MAT, SUD services, education around harm reduction, etc.

Supportive Skills Training

  • Supports program offerings of group skills training opportunities and facilitation of opportunities.
  • Assess the functional level and program needs of each participant, providing skills training as indicated, and support to staff in their role.
  • Provides support in milieu setting as indicated and opportunities arise.

Documentation/Expectations

  • Completes documentation and data entry as required by contract (i.e., Electronic Health Record and HMIS/ServicePoint system, etc.) within 48 hrs.
  • Utilizes centralized scheduling to account for client appointments, meetings, training, etc.
  • Utilizes outcomes-based care tools to provide outcome informed care.
  • Ensures timely and accurate completion of all required paperwork.

Description of other duties:

  • Performs other duties as assigned.
  • Participates in On Call rotation, which includes potential for mobilization.
  • Provides support to teammates and fosters a culture of inclusion, trust, and safety.
  • Completes all other assignments in a timely manner as requested by supervisor.
  • Maintains a regular reliable work schedule, which may include day, swing, overnight, or weekend shifts.

  • Follows, complies, and adheres to all directives and instructions.
  • Maintains CPR and First Aid Certificate.
  • Must be able to pass a criminal history check through the Department of Human Services.

Qualifications:

  • Minimum two years of providing direct care experience in community-based setting with population served (SPMI)
  • Preferred: Qualified Mental Health Associate credential
  • Preferred: CADC
  • Preferred: Prior supervisory or lead experience.
  • Premium offered to candidates for bilingual skills.
  • Incumbents with a substance use disorder must be sober and free of substance use for 2 full years preceding date of hire and must maintain this sobriety.
  • Must maintain current Oregon or Washington State driver’s license and insurance. Must have own reliable vehicle to meet participants in the community, as the position requires some driving.
  • Must have a high school diploma or equivalent.
