Case Management Assistant
The Case Management Assistant is responsible for providing accurate andtimely administrative and technical support to the case management teams,helping ensure a positive patient experience and effective coordination of careand utilization review processes. Responsible for assisting in the dischargeplanning and utilization review process for all patients and accuratelyentering patient data and documentation into records systems while interactingwith doctors, nurses, insurance agencies, and patients to bring all aspects ofa patient’s care together and to match identified patient needs to theprofessionals who can meet them. This includes coordinating all dischargeplanning requests with Case Management, verifying insurances and obtainingauthorizations as necessary, working with internal staff to determinepreparedness for outside appointments and discharge.
Minimum Qualifications
• Two years’ experience in rehabilitation orclosely related field.
• 2-3 years of direct patient care or healtheducation in an inpatient, skilled nursing, home health or related settingrequired.
• Desired Data entry and experience inpre-authorization
• Experience coordinating benefits and reviewingalternatives with patients and families. Experience in healthcare settingworking directly with interdisciplinary team/patients/families in a clinicalteam setting strongly preferred. Knowledge,
