For more than 30 years, SCCS has provided care, counseling, coaching, and essential mental health services for children, teen, young adults, and their families. As specialists in the field of mental health, SCCS employees possess diverse professional skills equipping us as a team to treat each client’s unique needs.

As a Bilingual (Spanish-Speaking) Mental Health Care Coordinator, you will be responsible for the overall coordination of care for referred clients and their families. You will coordinate and facilitate meetings and guide the evolution of an individual and family-centered Plan-of-Care that effectively promotes a successful home placement for the referred youth by utilization of the family’s natural and community resources. You will maintain the quality of care, implement special services, and complete all related documentation.

What you will do:

  • Maintain a caseload of eight to ten (8-10) slots with an average of fifteen to sixteen (15-16) hours of service contacts per month per Participant or Participant’s Family Team. The fifteen to sixteen (15-16) hours include telephone contacts, face-to-face contacts with the Participant or the Participant’s family, consultation time as necessary, case management and documentation, and identified crisis time as described below.
  • Coordinate the meeting with the Participant’s Family Team and develop the individual Plan of Care based on the Family Team’s strengths and needs. Modify the plan as needed.
  • Assist the Participant and the Participant’s Family Team to access strength-based mental health, social services, education services, and other supports and services as identified by the Participant and the Participant’s Family Team including services available through the Provider Network.
  • Provide or secure support and crisis/emergency services for the Participant and/or the Participant’s Family Team, including services available through the Provider Network.
  • Be available by cell phone 24/7 for contact by the family/client per county contract.

Qualifications

Requirements:

  • Bachelor’s Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university.
  • One (1) year experience in human services, preferably case management; One (1) year of experience working with target population as defined by the contract.
  • Bilingual (Spanish-Speaking)
  • Completion of CPR/First Aid certification within the first thirty (30) days of employment
  • Valid CA driver’s license and proof of automobile insurance.
