Description

The Re-Entry Housing Navigator works with the Peer Support Specialist, and other support team members to support individuals with a justice involved background and experiencing houselessness to prepare for, acquire and retain housing. The purpose of this program is to address the gap in housing services for individuals who have become homeless due to involvement in the criminal justice system, as well as connecting the individual to employment opportunities. The Re-Entry program is a 24 month program, services provided by the Housing Navigator include skills training, housing plan creation, identifying appropriate housing resources, providing ongoing retention support (alongside the Peer Support Specialist), establishing community resources to promote long-term housing retention, and connecting participants with employment prior to graduation from the Re-Entry Program.

Essential Duties and Responsibilities

  • Take the lead on accepting referrals to the Re-Entry Program, executing intakes, and initiating meetings with participants
  • Execute Coordinated Entry assessments in alignment with the standards of Washington County’s Continuum of Care system
  • Assist with practical tasks necessary for establishing housing, including development of a housing plan, addressing screening barriers, creation of a housing portfolio, submission of wait list and rental applications, reviewing and completing property management compliance paperwork, lease agreements, etc.
  • Work with individuals in the target population; engage those who meet Re-Entry Program eligibility through both individual and group interventions
  • Act as main contact between Re-Entry Program and Washington County Department of Housing Services for implementation of rental subsidies
  • Assist individuals in applying for subsidized housing waitlists
  • Meet with program participants in the office, their homes or other locations in the community
  • Develop rapport with participants and utilize personal and professional experience to promote wellness and autonomy
  • Assist in the housing exploration process, including housing search, meeting with prospective property managers, coordinating application processes with property managers, appealing denials, etc.
  • Enhance opportunities for individuals to become familiar with and utilize community resources including, but not limited to: food pantries, grocery stores, public transit, emergency financial assistance, mental health and medical services, sobriety support groups, vocational and educational opportunities, churches, community centers, etc.
  • Assist Peer Support Specialist and participants in creating housing retention plans that identifies their self-determined goals, employment goals, long-term housing goals and specific objectives to carry out these goals
  • Acquire training to teach Rent Well, periodically offer Rent Well classes (assisting Peer Support Specialist), and support individuals in completing the course
  • Support individuals in identifying and applying for subsidized housing opportunities and to maintain their status on housing wait lists, as applicable
  • Attend and positively participate in required team, agency, and interdisciplinary meetings
  • Coordinate and consult with other team members to keep team informed of issues and progress
  • Participate in bi-monthly case conferencing of participants along with staff at the jail, community corrections facility, and vocational rehab workers/employment specialists
  • Ensure timely completion of all required paperwork including charting and tracking progress of participants, with the goal of seeking outcomes for housing retention, employment and increased income into the Homeless Management Information System (HMIS)
  • Work to increase level of cultural sensitivity, awareness and competency regarding person-centered services
  • Maintain a regular work schedule
  • Complete all other assignments in a timely manner as requested by supervisor

Supervision Received

  • Receive regular weekly supervision from the Re-Entry Program Manager, and seek additional supervision when necessary
  • Participate in interdisciplinary team meetings and group supervision as required

  • Maintain CPR and First Aid Certificate

Must be able to pass a criminal history check through the Department of Human Services
